Pay attention to the death message you receive when you 'get killed'. You'll notice something different.

I personally don't know, when I get killed by the wright I know I get killed twice, I dunno though if those deaths count as "real" deaths.
What Celerity said. If you land at your pit naked, then yes. This can happen if you are taking damage from hunger / thirst / drug addiction or are a raged serk.
Therefore, the answer is no.
